The reason it is sporadic is because it is condition dependent. A lazy injector is allowed a +4mg/stroke of fuel adaptation before triggering the error. An oddly leaky injector will be pulled to -5mg/str before triggering a different error.
This was my cyl #4 before and after putting in a new injector and coding it. Result was perfect balance again

How did you calibrate injectors 1, 2, and 3 to get tighter values? Or did they just come in after #4 was replaced?
