In my opinion two issues make a huge impact on reliability: (1) complexity and (2) how new are the technologies in the car. Luxury cars, like BMWs, tend to have lots of complex systems and the more there are, the more opportunities for something to go wrong. One reason that Toyota and Lexus rate so highly on reliability is that both tend not to bring in new technologies as fast as their competitors so their cars are not as cutting edge but are therefore more reliable.

Yeah I agree on Toyota's conservative approach, which tends to be more reliable at the cost of not being "exciting" enough and lagging behind in terms of the german competition.

As a first timer BMW owner, it is comforting to see the company trying to shake the unreliability stigma that is/was synonymous with German automobiles.
